 In order to participate in off-campus study, a number of mandatory forms must be submitted before, during and after a student's off-campus experience.  These forms include the Application for Approval to Study Off-Campus, the Transfer of Credit form, medical forms, and Assumption of Risk forms.  Students must also seek the approval of their academic adviser and recommendations from two professors.  You must have Adobe Acrobat installed to download these pages.
