Individual Teaching Consultation Request Form

The faculty director of the Center will meet individually with faculty who request a personal teaching consultation.  Or, if more appropriate, the faculty director will arrange a personal teaching consultation with one of the members of the Faculty Advisory Committee on Teaching or another experienced faculty member. 

Please describe in some detail what areas of teaching you would like to discuss.  Your response here will help the faculty director determine the most appropriate way to help you achieve your goals.
