There are no member ID cards for the VBA plan. When you are ready to use the benefit, simply request a Benefit Form from VBA, by visiting the VBA web site or calling (800) 432-4966. VBA will verify participant eligiblity and send the member a benefit form, along with a current list of participating providers. Present this benefit form at the time of your visit, and the participating provider's office will handle the rest!
