This plan covers loss of life, limb, hearing, sight, speech and other losses due to an accident.  There is coverage for both you and your eligible dependents.  Coverage terminates at retirement.  There is no conversion option with this plan.

Benefit maximums*:

Employee: $100,000

Spouse/Spousal Equivalent: $50,000

Child(ren): $10,000

*Benefits are pro-rated for loss of limb, hearing, sight, speech and other losses.

Rates: 

Employee: $2.50 per month

Employee & dependents: $4.20 per month (regardless of the number of dependents)

"Dependents" are the employee’s spouse/spousal equivalent and all eligible dependent children (less age 19 and dependent on the employee for support, or less than age 23 if enrolled as a full-time student).
