Medical History Record

All new students are required to complete the medical history record for the Student Health Center. The form is sent to you in May along with your registration materials. You will need to see your doctor to have the form completed and medical dates verified. It must be completed in English and returned in the envelope provided to the Student Health Center.

If for some reason you are late obtaining this information during the summer you can bring the document with you to International Student Orientation. A session is included during this program for you to meet with the Student Health Center Staff and to verify that Bucknell has your medical record.
