As part of our caring community, we all play an important role in looking out for one another and ensuring the well-being of our students. Bucknell has many resources available to support all members of our community. If you're concerned about a student — or if you, a student, need support — please don't hesitate to share your concerns with us. Students, faculty, staff, family and community members can use the forms below to request outreach and support for students, as well as to submit reports of concerning behavior and violations of the Code of Conduct; the Equal Opportunity, Harassment and Nondiscrimination Policy; Employee Handbook Policies or academic integrity.
Student Support
Refer to CARE Team
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Referring students when there is a concern for harm to self or others. Contact the CARE Team at 570-577-3319 with any questions.
Student Outreach and Support
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Referring a student who is dealing with a challenge or concern and may benefit from connection to campus resources and support.
Report to the Office of the Dean of Students for Outreach and Support
Request for Pregnancy and Related Conditions Accommodations
Can be submitted by: Students
Used for: Students requesting reasonable accommodations for Pregnancy and Related Conditions.
Academic Concerns & Accommodations
Student Academic Alerts
Can be submitted by: Faculty, Staff
Used for: Referring students to the academic associate dean of their college when there is an academic concern, i.e., attendance issues, lack of participation in class.
Reporting Suspected Academic Misconduct
Can be submitted by: Faculty
Used for: Reporting behavior by a Bucknell student that may violate the Academic Irresponsibility policy. This report will be sent to the Associate Academic Dean of the student for review.
Student Accommodations
Can be submitted by: Students
Used for: Students seeking academic or other accommodations within Bucknell through the Office of Accessibility Resources.
Misconduct
Report Student or Organizational Misconduct
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Reporting a possible violation of the Student Code, in which the violation was committed by a Bucknell student, on or off campus. Do not use this to report sexual misconduct, relationship violence or stalking; please use the form under "Report Sexual Misconduct."
Report Sexual Misconduct, Relationship Violence & Stalking
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Reporting sexual misconduct, including sexual harassment, sexual assault, stalking and relationship violence allegations that may violate Bucknell Policies. Anonymous reports can be submitted.
Report Study Abroad Incidents
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Informing the Office of Global & Off-Campus Education and other Bucknell partners about incidents that occur abroad. Information will be reviewed and appropriate parties will follow up.
Report Hazing
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Reporting alleged violations of Bucknell University's Hazing Policy.

Report a Complaint

Used for: Reporting to the Dean of Students Office informal complaints regarding Bucknell that do not fall under any other Bucknell policy.
Report Damage to Housing Facilities
Can be submitted by: Students, Faculty, Staff, Community Members
Used for: Reporting an incident of damage or vandalism in a residence hall common space that requires restoration or repair.
TMA Work Order
Can be submitted by: Students, Faculty, Staff
Used for: Request work in residence hall rooms, offices and buildings by submitting a work order through the Facilities work order system, TMA.
